Documents and Forms

Go to the Scouting Forms from the National Council web page for youth and adult applications (in English or Spanish), tour plan forms, medical forms, uniform inspection sheets, adult volunteer progress records, Journey to Excellence forms, and more.

Council & District Event Forms

Go to the council online calendar, click on the event, then click on the more details link. If there is a registration form associated with an event, typically, its link is available from the event details page.

For training events, go to the adult training courses or youth training courses pages. If there is a registration form associated with a course, its link is available with the course listing.

Council Summer & Year-Round Camping Forms

Go to the Camp Squanto website for all Summer camp-related forms for your unit. Note that the NYLT program and the Eagle week program have websites with all the information scouts need for success in these programs.

For year-round camping activities, use the following forms:

• Tour Plan Form (required for all tours). Or, use the online Tour Plan system. For more info, see the Tour Plan FAQ.

• Float Plan Form (required for tours with aquatic activities)

• Camp Squanto Site/Cabin Reservation Form (required for unit weekend camping). Here are directions to camp and a map of camp.

• Camp Squanto (off-season) COPE Reservation Form (required to schedule COPE program activities). Also, this program requires an Informed Consent Form for each participant.

• Camp Squanto Facility Reservation Form (required for District/Council camp-wide facility reservations, including the dining hall, program areas, and sites/cabins)

• Camp Squanto Campership Form (submissions accepted between January 1st and June 1st)
